Patient Centered Primary Care Collaborative Releases Two Reports on Medical Homes

The Patient Centered Primary Care Collaboratice (PCPCC), of which the National Council is a member, has released two new reports with information and resources on medical homes. “The Patient-Centered Medical Home: Integrating Comprehensive Medication Management to Optimize Patient Outcome” provides information about maximizing the appropriate use of medications to prevent and control disease. “Payment Reform to Support High-Performing Practice” analyzes the strengths and shortcomings of available medical home models with the intent of providing advocates and providers with tools to select the model that best suits their needs.
